Person & Family Centered Care
At Kaiser Permanente, Person and Family Centered Care (PFCC) has redefined the relationships in health care by placing an emphasis on collaborating with people of all ages, at all levels of care, and in all health care settings. As an organization, we recognize that the outcomes for quality, safety, and delivery of care are improved when the clinical expertise of health care providers is partnered with the lived experience and expertise of patients, families, and caregivers. The patients, family members, and caregivers who have chosen to collaborate with Kaiser Permanente on an on-going basis are called Patient Partners.
Kaiser Permanente engages with over 1300 Patient Partners across all regions who are actively involved in helping maintain Kaiser Permanente’s position as a leader in providing high quality and person-centered care. Across the enterprise, there are over 90 Patient Advisory Councils (PACs) that focus on a range of areas including local medical center operations, mental health, autism, NICU parents, teen, transgender, oncology, and more. While most PACs are English speaking, we also have several Spanish, Chinese and Tagalog speaking PACs.

Kaiser Permanente's Person-Centered Care results from trusted partnerships among patients, families and care teams, where providers listen to and honor patient perspectives and choices. Patients are supported in setting personal health goals and in engaging in shared decision making at the level they choose. Care processes are designed and continuously improved based on quality outcomes and the experiences of members, families, clinicians and staff.
